Metal adapter

Hi, I am after a metal plate to convert 4 small screw holes to a different location where I can screw others. Attached are pictures with hole dimensions. Please ask any questions as i probably haven't explained it well enough. Thanks

Please see attached a drawing of what I am after the holes around the outside need to be tapped and ideally M6. The 4 holes in the centre on the back need to be countersink to accept hex bolts and need to be around 1/4 to allow a 1/4 threaded hex bolt to fit though.
