VERY simple part, can be produced by means other than milling if it is easier.
Material: Clear Polycarbonate
The component is mostly flat, 5mm thick, but requires 4 holes at ~3.8mm diameter (tolerance unimportant) and a squared recess of depth 0.75mm, width 1.7mm (tolerance p/m 0.15mm). Avoiding the holes and recess intersecting would be ideal.
